Simply Print, Sign and Submit: 

  • Print the ready-to-use letter.

  • Enter your child’s name, grade and sign at the bottom.

  • Check off the specific rating types (3, 4, 5) and sensitive content you wish to be notified about.

  • Give the document to your school principal or librarian to be placed in your child’s file.
